R-002. Drug: thalidomide. This trial will study thalidomide for the treatment of aphthous ulcers. Aphthous ulcers are mouth ulcers with no known cause. Participants will be given one of three doses of thalidomide for 28 days. Those participants whose ulcers respond to treatment will then be randomly placed in one of two groups for the rest of the study. One group will take 100mg of thalidomide a day to see if it stops the ulcers coming back. The second group will take a placebo or dummy pill. This part of the study is called the maintenance phase, and will last for up to one year. Participants whose ulcers don't respond to thalidomide will leave the study and receive appropriate treatment. Neither the doctor nor the participant will know which treatment is being taken. To participate in this study you must be HIV+ and 13 years or older. You must be on stable treatment with at least two anti-HIV drugs for one month prior to study. Women who can have children must either commit to abstinence or use two methods of contraception, one barrier and one hormonal. Total time on study is up to one year. You cannot have ulcers due to Herpes simplex virus. You cannot be taking systemic corticosteroids within 14 days prior to study.
