Requirements

Applicants must have exhausted other avenues of funding and have no prescription drug coverage. Information on status of applications to ADAP or Medicaid programs is required. Financial information is also requested, limits on earnings are not specified but are well above the Federal poverty guidelines. A doctor can enroll a patient for a 90 day period which can be extended with certification.
